Spanning Systems for Change

The goal of CarePlus Transition Services is to identify consumers and provide mental health interventions that prevent incarceration, or decrease relapse or recidivism.

Working in collaboration with consumers, the hospitals, mental health providers and the Bergen County Justice System, Transition Services strives to:

  • Increase the consumer’s ability to manage his or her mental health needs within the community, rather than incarceration
  • Enhance the consumer’s ability to re-integrate into the community with increased support via case management
  • Identify possible service gaps and break down barriers that impede the consumer’s ability to maintain stabilization outside the legal system
  • Participate in the development and implementation of a community service plan with our consumers and their family members to increase tenure outside of the correctional system and enhance those opportunities that increase a positive quality life
