Outpatient Recovery Treatment & Care Services
While our clients come to us at various levels of recovery readiness, our clinical and peer support programs have shown to be effective in helping to move you forward, and to maintaining a healthier outlook and long-term sobriety. To ensure we meet the needs of our clients, we offer services for individuals and families, as well as intensive outpatient services for those who require it.
Integrated Case Management Services
For those individuals who have recently discharged from either a psychiatric hospital or inpatient substance abuse program, CarePlus offers case management services. We help ensure that walking out the doors doesn’t mean the end of your treatment programs. Our team will work with you to ensure you get the care and services you need to maintain recovery and pursue rebuilding your life. That means discharge planning, establishing appropriate linkages to mental health providers, utilizing available community resources, advocating on your behalf, and many other services.
