Position: Screener
Department: Psychiatric Emergency Screening Program (PESP) Hours: Full Time, Part Time and Per Diem hours available. Qualifications: Master’s degree in a related field and 1 year mental health experience; OR Bachelor’s degree and 3 years mental health experience, one of which must be in inpatient setting, Screening center or emergency service; OR Bachelor’s degree plus 2 years of paid experience in the mental health field, one of which is in a crisis setting and enrolled in a Master’s program. Eligibility for TAC temporary screener certification. Valid Driver’s License Job Summary: Provides hotline intervention, psychiatric assessment, therapeutic crisis intervention and when applicable on-site and outreach, provides treatment planning, crisis intervention, follow-up and linkage services. Valid Driver’s License Job Summary: Provides individual and group counseling both on-site and at the group homes located throughout Bergen County; assist clients in Activities of Daily Living; provides financial and benefits assistance; provides case management services; prepares a comprehensive treatment plan; facilitates socialization activities; responsible for Rental Assistance Program; assists with the acquisition of transportation and transports clients on a daily basis; 90% of time is dedicated to direct services. Job Summary: Positions available in our group homes. Primary responsibility is to supervise adult mentally ill consumers in a group home setting that includes medication monitoring and ADL's. Job Summary: Responsible and accountable for providing direct behavioral health care services for an adult person with severe and persistent mental illness (SPMI) in an outreach/case management program. Job Summary: *ONE YEAR PROJECT* Acts as primary Outreach Case Manager for adults receiving TANF, who have unexcused absences in their WFNJ assigned activities in the previous month. Outreach efforts will include phone calls and home visits to determine if the client has been re-engaged in their assigned activities. Individual will maintain a caseload per assigned by the Director. Duties will include the completion of the Comprehensive Social Assessments, PRI summary report and monthly reports. As per State Contract duties, will also include at a minimum referrals to appropriate entities for job development, client advocacy, case management and service coordination, linkages to community resources, substance abuse/mental health care services, medical deferrals and/or alternative benefit programs.
